About HCR Cares

HCR Cares is a donor-funded 501(c)3 agency incorporated in 1978 and is a United Way donor designated agency. HCR Cares is not affiliated with HCR Home Care, but often partners on HCR’s programs and initiatives. HCR Cares has had notable impact on home health care in New York State and across the home care industry.

Why C.A.R.E.S?

Collaboration

Partnering with HCR Home Care, a certified home health agency, and other community and foundation partners, HCR Cares identifies the health care needs of the times and responds with research, innovative strategies, and educational opportunities.

Activities

Falls cost our health care system $4 billion annually. Research by HCR Cares developed, tested, and disseminated a Falls Prevention Program that establishes a culture of safety and provides group exercise to reduce falls and fall-related injuries. This initiative is quickly making Rochester a Center of Excellence for Falls Prevention.

Research

With a grant from the Greater Rochester Health Foundation, HCR Cares addressed health disparities in Hispanics, identifying culturally competent practices that promote improved health management. As a result, patient outcomes were greatly improved.

Education

To address our area’s nursing shortage and lack of diversity in the nursing workforce, HCR Cares provides Nursing Education Grants to employees of HCR Home Care to further their careers by becoming a licensed practical nurse (LPN), registered nurse (RN), or BSN. Mentors also support the grantees and help with time management and study skills to ensure success.

Socialization

HCR Cares brings community leaders together and elevates best practices in Rochester by sponsoring educational seminars presented by scientific experts and leaders in the health care industry. In a grant from the Robert Wood Johnson Foundation, HCR Cares demonstrated how community collaboration was the best way to identify and reduce barriers.
